Canadian here. By what standard / and what type of health work? I'm not in the sector but have friends in these practices across 3 of the 4.
Overall I'd say Deloitte leads on HealthTech / Platforms. KPMG is the clear winner in depth across strategy and operations. EY is building, and has won some interesting work but (imo) I haven't heard the teams impress with quality results yet - but it'll come over the next 3 years with the senior leaders they've picked. PwC's is smaller, and does well in strategy (system transformation, strategy) and is rebuilding the operations side based in some job postings. PwC is limited on tech but has a strong Life Sciences / MedTech practice focused on M&A / Market entry on the private side.
I'd say the clear choice is KPMG on paper, but they were absolute jerks during their campus hiring process and I can't respect them for treating students that way.

I have had some luck with trying to speak to people via coffee chat/information interviews that previously worked at my target company to get more candid feedback about their experience. Check on LinkedIn for people who previously worked at one of those firms and have left.
