Can someone here help with how the variable/bonus payout is carried out at Visa? My HR told me it varies between 0-250% of the variable amount.

If your variable is 12% it usually means - Your 100% bonus is 12% of your fixed pay.
Your bonus can range between 0 and 250% depending on your performance and the companies metrics.
For example if 12% of your basic pay is 5 lakhs.
If you get a bonus payout of 100% you’ll get 5L
If you get a bonus payout of 250% you’ll get 12.5L
