If you want some photos showing some of your work put that in a portfolio.
Not for anything that needs to be parsed by an automated system - so basically all online applications.
Any US company will also immediately toss out your application. Almost all have a policy that they won’t accept headshots since seeing applicant photos leaves them open to discrimination lawsuits.

Nay, but I can see it depends a lot on your field. More creative roles can potentially get away with it, I’d lean towards not including though even in that. If you really wanted to see a picture of me, would not be that hard to find one out there so I try to just ignore the idea of biases based on it!
Dumb idea. Smart of you to ask, but not professional.
Bad idea. First of all, anything that takes up space that could be text or white space is a bad choice. And there are any number of other problems. If you provide your LinkedIn handle, people can get a look at you there.
