You need to tread very carefully in how you approach the manager. Don't ask them to change the decision. If you must reach out, word it along the lines of "I enjoyed meeting with you, and even though it didn't work out this time I hope you will still consider me for future opportunities." I've had candidates reach out to me after a rejection still insisting that they're the perfect fit for the role, and that disqualified them from any future consideration.
Not at all, go ahead and send it.

It depends on a TON of factors. Why were you rejected? How far along are they in the process? How much decision making power does the hiring manager have at this stage? Etc.

I would say “thank you” in an email and let it go. From my experience, I think they consider it pestering
