Because sometimes the pain is too great to not blurt out something.
Usually this stems from an individuals need to "create value" because they feel that their on the spot. And as you have already stated, this usually doesn't lead to anything useful to start. It can be a spring board for new ideas that could make a difference though.
Or they just don't care.

I feel like they become extremely uncomfortable during a brief pause after someone asks them a question, so they just start speaking without thinking, rather than taking a minute to think before answering.
Not everyone is comfortable in meetings, sometimes this is just a stress behavior.
Perhaps people have different communications styles and they aren’t trying to filter what they say. In some cases, this is good - generating ideas, nothing is stupid. When you are looking for honesty, it is good.
When you need to reassure a customer that you have things in hand, it can be bad
