Yes. I did it on the way out the door from a big 4. The head of the practice was having an affair with a 24 year old freshly minted associate… who kept getting promoted every 6 months. It was a total joke. Everyone knew what was going but looked the other way. It was disgusting.
The 24-year-old was such a nightmare. I had people on her engagement team, calling me begging for confidential help on how to handle the situation.
It was stuff from 1950. She would scream at them when she had to take a few days off that “the partner needs his breakfast brought to his hotel room by 8 AM a blueberry smoothie and a green tea no sugar!!! No sugar!!!” And other crazy crap like this.
I dropped a dime on both of them when I left via the confidential hotline. He isn’t the practice head anymore and I would like to think I played a small part in that.

Had an incredibly sketchy experience on a large pubco audit. Left the firm due to not wanting to be associated with that team.
Within the year, 80% of the team was replaced, including the partner. I think 4 of 5 of the managers/senior managers on the file left the firm entirely.
Sad but honestly probably for the best.
Rising Star
Years ago at a previous firm. Filed a complaint against a super toxic PPMD-level person who would regularly yell at and verbally berate us SM-level folks who reported to her. Tolerated it for a year then finally had enough when a colleague had a nervous breakdown after a particularly abrasive and abusive meeting.
Filed an ethics complaint and all of us SMs were interviewed by the ethics team...then we never heard anything about it again. She was the "golden child" at her level, so my assumption is that the powers that be swept it under the rug.
I refused to work with her again and stayed at that firm for another year or two before moving on, in no small part due to that whole saga.
I did for one PMD and firm always protect them , nothing happened and case is closed .
HR is. Not. Your friend.
Nothing gets done, firm protects itself against you.
It's a 3rd party company that fields these reports, but I get your drift. It's more a protective measure, I suppose. I was forced to do something that I felt was wrong. I'm hoping this provides a bit of protection
