Yes, and it's a must when you're taking a risk, or the company you're going to could potentially lay you off, you're relocating, or are slightly more established in your career and even when finding a new position (if they lay you off) might take a while (market conditions softening), For instance, it's not uncommon for executives to negotiate 6mo, or 12mo severance. Also would suggest adding that they would cover your full COBRA healthcare premiums for several months.
C-suite and EVP Contract yes. Offer letter, rarely.
That’s usually part of an employment agreement. Far more detailed than an offer. Ask for an employment agreement as part of the offer

I had my husband negotiate a 1 yr severance in his offer letter because he was leaving a company where he had built up 15 yrs of tenure. Surprisingly his new company accepted but it was a negotiation point once salary couldn't go any further. It was put in writing in the offer letter and was negotiated with his would-be manager, not with HR

I would make sure to take into account the following
— a number of years exp &
how long you’ve been at your current role.
— if they (new company) headhunted you vs you reaching out to them.
— what the market is like for your role, how long would that t likely take you to find a new place if things don’t work out (I recruit for creative, so I know these roles are falling out of trees).
For a C-level role, I’d initiate an ask for 6months, or then a minimum of least 1month per year of service.
