I have had a telling to about my attention to detail. It’s only been a problem with one partner, he’s generally demanding and I always get in my head about stuff working with him. Also a 5th year. I say this to offer some consolation that you’re not alone, and the wisdom I have gained that the more I obsess over mistakes (after they are made), the more mistakes I make. I am working on slowing down my work, but moving on fast from the mistakes
I think you are in your head too much.
If all your feedback is good and no one has said anything to you about your "mistakes", then it is almost certainly nothing to worry about. People, even experienced lawyers, make mistakes. It is understood and expected.
Obviously, try your best not to, and set up systems to quickly detect and correct mistakes when they are made. But, other than that, we are all human. If it was a big issue, partners would not hesitate to let you know.

Depends on impact to client and the attorney supervising you.
I think it very much depends on your supervising attorney. I've had bosses who didn't care about anything I wrote, but lost their shit if I didn't hande something during a court appearance the way they would have handled it. I've also had the opposite.
