You might as well apply and then bring up those concerns if you're interviewed. Some places are very rigid about that kind of thing and it's like working in some kind of weird panopticon. If you have to be in the chair and using a mouse jiggler to keep the job it's probably a pretty miserable experience.
Something good to ask about further in the process. I have worked at placers where stepping out for a couple hours for an appointment wasn’t allowed. You would just have to take off the whole day and use up PTO or something for it.
They do expect you to be in the office 9-5, but you get 10 days per year for exceptions. Everyone has to be in on anchor days, and you pick your regular third day. If you wanted to work from home on one of those three days, you would get an exception from your manager to come in a different day that week. Disclaimer that this is how it works for most people, but it may be different if you have a disability or are pregnant, etc.
Strict 9-5 is false. On your 3 required in office days, you may start no later than 9am and must work a minimum of 8 hours. Additionally, you're allowed exceptions and flexibility, in my experience.
