It's best to use a real example from your work. We've all made mistakes that we rectified in real time, and those would be the ones to talk about. The purpose of the question is to see how you think and how you react to problematic situations. It's also to assess your honesty, so if you tell a story that doesn't seem plausible you'll be undermining yourself. Best to stick to a real life example.
Providing context helps A LOT. I once screwed up scheduling a flight for my boss because he'd said one date initially, I'd confirmed, and then he emailed saying he needed to change it. I was on my phone and must've clicked into it without noticing, because I never saw it and the thread showed as unread, so I assumed everything was good to go (it wasn't). I've told that in an interview before, explained the exact circumstances, and then the safeguards I've put in place to make sure it doesn't happen again. I don't think it's been held against me at all.
Just be honest. Explain the mistake (we all make them), but focus on how you rectified it. Don’t pick a “mistake” that makes you look good. Tell them you owned the mistake and learned from it.
Subject Expert
I mention a time when an executive asked me to schedule a meeting while I was in the middle of another time sensitive request. I told them I would do it after I wrapped up the current task. Unfortunately, I didn't write down the request and it slipped through the task as I focused on the initial item I was working on. Later, my exec followed up on the status and I owned the mistake right away. I took the time to immediately work on scheduling the needed meeting. I learned the importance of capturing every request, no matter how small it may seem, in the moment. Whether it is writing it down, flagging an email, or inputting it in to project management software I like to use. This has helped me not only stay on task with the numerous requests coming in, but also serves as a reference to follow up on things that might be needed
