All I've ever had to say was this:
"I've had some personal stuff come up that requires my attention. I'll be out of the office from x to x. I'll be able to check my email periodically, but will not be immediately available to respond. I'd be happy to fill in someone and get them up to speed to act on my behalf in case of emergency where immediacy is needed. Sorry for any inconvenience."
If you go the "I'm sick" route be prepared to send in a doctor's note just in case
Get a splitting headache, so bad that you cannot open your eyes. They may not ask for a doctor's note for just a headache. And then you miraculously recover after two days. In fact, you could work in the morning, get a headache, and disappear.

It's what, two days, maybe one, tops? Get sick, don't worry about the doctor's note. Don't bother taking PTO, especially if they might pay you for unused days.
yeah food poisoning is the best bet. not sure if your part of publicis/marcel😂 but a note is only required after 3 days.

I'm afraid of calling in sick because it's a 3-day on-site and I've never actually taken a sick day that didn't involve working from home. It seems like mysterious family death would provide me a lot more leeway, right?
I wouldnt want to say someone died who didnt as it feels icky. Just say a family situation has come up and you are going to need to take off x days. You've earned the PTO and it's summer. Take the days. Stay off social.

Maybe I'm too selfish but I'm 99% sure I'm going on the same interview as you, am currently on a 2 week trip in Europe and am gonna lie and say my plane got stuck for the extra 3 days off on my return. You do you dude. A dream job is a dream job.

My agency requires a copy of the funeral program for death in the family (3 days paid?). HR told me my grandfather was handsome. 😵

2 weeks notice for 2 days max of PTO is more than acceptable. Your place on the totem pole, or your shop being boutique, or your shop's level of busyness shouldn't have a goddamned thing to do with you being able to take your own personal time that you earned.
If you feel like you have to lie about it to get the time approved, claiming "family emergency" is an excuse that absolutely should not be questioned, and if it is, fuck 'em, it's your business, and you don't need to explain your personal matters to them.
If you just don't feel right about scheduling planned PTO, or fear the judgment and/or accusations, then just get "viral gastroenteritis," commonly called "stomach flu/bug." It's basically something a doc can't do much about, so you just end up being glued to a toilet bowl while you shiver with fever, shitting and puking your guts out for 1-3 days. And then when you're done evacuating every bit of liquid from your body via mouth and butthole, the next day, you're a limp pile of over cooked spaghetti noodles with a massive throbbing headache because you're dehydrated, and you're weak from being malnourished, so you need a day of recovery to rest, eat, and replenish fluids.
It's better to claim "stomach bug" than "food poisoning," because, while the symptoms are similar, the latter runs (lol, pun intended) its course faster, so most people aren't out from food poisoning for more than a day or two. Whereas you could conceivably be out for stomach bug for 1-4 days. Maybe longer.

Also it's two weeks out! seriously, how slammed is your place? Put in the request now. They have two weeks to schedule around it. Unless you are personally the lead AD on a shoot or something that's the same day as the interview, you should just tell them tomorrow.

Just an FYI, y'all make great points about taking PTO on your own terms and stuff, I totally agree -- but I work at a boutique shop and "sudden" absences in the creative dept can totally fuck people's week's up. When there are 4 other people taking vacation in a given week, I feel like it's fair that I can't also decide to vacation that week. Am I being naive here?
No. That's why sometimes it just needs to be an "emergency." It's not like you're skipping town for a buddy's bachelor party, this is your career and your life. They can always get a freelancer if it's that much of a problem.
