When I need to take time off, I usually communicate with my supervisors in advance. I think it's best to let them know as soon as possible so they can plan accordingly. I typically send an email to my direct supervisor with the dates I will be out of the office and the reason for my absence. In your case, you may want to mention that you will be taking a week off in June and provide any relevant details.

I always give my boss written notice if I need to take time off. A smooth return to work and a successful vacation can both be attributed to effective communication. Therefore, you should be truthful and inform your employer of your plans.
Just tell them - it's a demand. not a question! Don't give them the option to push back. Just saying, 'hey I'll be taking time off between x and y date, have checked my projects nad nothing will happen in the meantime etc'
Try to request time off when it is least disruptive to your team's workflow. If you work on a specific case or project, make sure to coordinate with your team to ensure that your absence won't cause any delays or issues.
I think it depends on what the nature of your leave is. If it's something personal/medical/urgent then I would be direct with your manager about the reason (as much as you're willing to share) that you need a week off. If it's something less serious, I would be strategic aobut how you ask/inform them that you need the time off. either way, i would have the conversation in person if possible.
Does you firm give you an PTO? If you're using your paid time off then you don't really need to go into detail about why you're taking off. You can just say a trip or for family
