I feel lucky when my assistant renames and saves a document accurately.

Same.
My secretary definitely always does that kind of stuff. And stuff I wouldn’t even know to ask for. But she’s like 60 yo and has been doing it as long as Ive been alive.
At this point, you should decide one person who is responsible for all that. Next time, I’d hire the 60 yo

Sounds like it’s at least partly a training and expectations setting problem, which is your responsibility…
As a legal assistant/paralegal with 30+ years of experience, it was always my job to make sure if a hearing was actually needed or not, no matter what the reason. Hadn’t run into a Judge at the State and/or Federal that didn’t make procedures perfectly clear.
As part of scheduling and filing documents, I always check the judge specific rules.
Always the attorney’s fault.
Yes, they should have caught that.

Without knowing their standard expectations that have actually been communicated to them, I feel like you would need to explicitly ask support staff to look for such a thing. It seems like a trick question. Like asking them to help you get ready for a hearing but the real answer is that there is no hearing needed.
If it’s Federal and they are used to State I understand as an assistant myself I’ve lear always to check regardless it’s common knowledge and practice
I'm one of those 60-year-olds and yes that's something I would do. That's how we were "raised." Unfortunately it seems like most firms don't want to hire the 60-year-old. So glad to see you appreciate experience!
You’re not wrong to be frustrated. It’s reasonable to expect a paralegal or legal assistant to check judge-specific procedures, standing orders, and admin orders before a hearing if that’s part of their assigned prep.
That said, under U.S. legal ethics rules, the attorney is ultimately responsible for supervising staff and ensuring the case is properly prepared. So this is probably both: a fair staff performance concern and a workflow/supervision issue.
Best takeaway: don’t frame it as blame. Create a required pre-hearing checklist and make judge procedures/admin orders a mandatory item before every appearance.
