We are in a case where my dad didnt get us passport even after passing the test and we speculate it might be because my dad worked on a tourist visa 25 years ago in the US. Is there any way we can get over this and get passport issued for him? Any advice

Got a GRE score of 320 (162V 158Q). Did my masters from A&M (full scholarship, 3.8 GPA, journal publications. I am an senior associate at a small (8-10 attorneys) immigration firm in NYC. My base is 110k and I get 30% for the cases and clients I bring in. My revenue was 370k last year and I did 250k till April this year.
