I obviously can’t comment on your specific company’s policy, but I can tell you that at my company (where I get more than 12 weeks) I was required to file for MA PFML for the full 12 weeks I was qualified for. MA paid me directly what I was owed through the program and my company made up the difference. So, I was taking home the same total $ but my company wasn’t going to foot the bill for the full thing when we’re all paying taxes into PFML. In your case I’d not be surprised if it’s the same for the first 10 weeks, then just PFML paying out in weeks 11-12. Could be worth asking if you’re not sure! Congrats 😄

It means your company is violating MA state law. You don’t have to take it all at once but it has to be completed within a year of becoming a new parent.
If your company is tiny or foreign (other US states) that might explain the gap between legal requirements and company policy. Also, this is basic stuff every people manager should already know.
