Honestly a couple. But the most recent one was me not being keen on a vendor's mistake. She wrote a really nice quote package for a new packaging line extension, which we agreed to, wrote our internal expense request. A day later she came back saying that she screwed up and the summary page was 100K different from the rest of the quote. Now my project is down 100 grand...
