Why can’t firms tell associates how to bill for things? Everyone dances around the issue it seems it would be easier just to train everyone on a standard practice. Is there some liability issue preventing this?

One simple reason is that different lawyers handle work for different carriers or TPA’s, etc. All these have a variety of idiosyncratic billing guidelines. No one size fits all billing guidelines the firms.
