There is the possibility that your comments are too vague or unclear about what you want done to have them cleared.
If it's a regular thing... Might be worth asking the people under you to see if there is some reason for it.

There is no might about it. You should definitely be asking. The issue they will likely run into is that they won't get an honest answer if they haven't invested in the working relationship.
As a manager, sometimes these people have no idea what they're doing. I just asked a guy about a credit for revenue. And he said the normal accounting balance is a debit. PA relies too much on open this workpaper and do it like last year.
Was the transaction dealing with returned items?
Find people you work well with and who respect you, and have them take care of it.
Then make sure they get a good review and bonus.

Schedule calls with them to make sure they understand the ask. These calls also send them a signal that you expect them to clear all the comments and you are not going to just take over and wipe their a*ses every time.
You need to walk through them on calls or have them shadow you. Or reverse shadow them (watch them while they clear comments - typically works better while both parties are in office). The point of this is to set clear expectations on how to clear comments according to what you want specifically. The staff are dumb dumbs in their first years and need handholding.
Another way is to develop personal relationships with them and take time to get to know people. People enjoy working for and with those who make an effort to both get to know them and support them.

Sometimes the best solution is to set up a call and go over your comments. Get a feel for how they are and if you get a sense of them not being sure elaborate.
What Partner1 said is ridiculous lol I’m sure they’re not a partner. Leaders need to know how to lead. Be a leader and they will follow. Even the worse staff can produce quality work
Maybe its the way you are handling your comments. Instead of writing up comments in isolation from your team and passing them to your staff, maybe you should sit down with your team and go through your comments and explain your comments to them and how to resolve or clear them. Maybe they don’t have the experience to deal with the comments and need further training, guidance and direction on what is needed from them so you can clear your comments. Work together as a team!
That is why you review by discussion. If they cannot answer you on the spot, means they have not done their work. At the end, the prepare a review memorandum for you to clear.
