How do you navigate getting a referral on a platform like this? Genuinely curious on if HR/Recruiting asks how you know the person you're referring, or if they don't care where the referral comes from.

Did you ask your contact for feedback on the interview or why you didn’t get the job? What folks say in the room vs what they tell the recruiter/HR person can be different.
Most of the time people give jobs to people they know. It's just safer not to risk that person having a personality clash with the rest of the team if they know them beforehand. So, seek the people you know to recommend you to places.
You might be bad at interviewing
That is good, but still doesn’t necessarily mean you’re hitting all the points you need to in the interview. Or following up with an emphatic and convincing thank you. I often meet with people who do a good job being personable, but somehow seem to forget to tell me any good reasons for me to fight for them to get a job
I tell everyone I don’t hire that they have a good book.
