If you said point-blank you were uncomfortable with an on-screen interview, that might be why. I appreciate leading with honesty, but it might have come off badly to the recruiter. You are potentially showing them you are inflexible. I don’t know for sure, but it’s something to consider.

Honestly, I don’t think it’s a mistake at all. You advocated for the format that would allow you to show up as your best self—and that’s a good thing. If anything, it shows self-awareness and a desire to build a real connection. If the company penalizes that, maybe that’s a sign in itself.
I agree with Square 1 above—I don’t see anything wrong with what you did. As a manager, I’d much prefer to do a face-to-face interview, and I’d welcome it if a candidate asked for one. I don’t think anything you did could be seen as a red flag.
I don’t think it’s wrong at all. It’s probably just a lazy recruiter who wants to do things that are easier. Probably dodged a bullet earlier in the cycle
