most widebodies on international routes should have the new Polaris seats now so yes they only fly out of hubs plus jfk and boston. Some narrowbodies to europe or hawaii have lie flat as well but it'll be the "fake" Polaris.
The issue will be finding cheap/saver award space for 50-75k one way depending on the route so you have to be flexible with dates. If you don't mind burning miles, everyday award is almost always available at 175k one way.
Yes, they only fly out of certain airports and on certain routes. Then within those routes not all planes will be ones with lay flat/delta one.
This has probably changed since I’ve looked it up… but a potential example to look up would be the jfk to Seattle route for delta. It used to be half the flights has delta one available the rest just had regular first class. You could try looking up these route to see an example.
