It depends. Have you tried following up? Some companies take forever to make a decision. I wouldn’t say three weeks is not all that long but I guess it’s not a great sign either.
Did they tell you how long it would take for them to get back to you? Three weeks isn't unheard of, but it's not a great sign. Send them a follow up and then cross your fingers that you get a response.
Yes and no. I mean Geico was about a month and 3 weeks in-between each interview & panel interview then decided "to go with someone's who's skills better fot their current vision." Ive noticed that if it's more than a month & no word move on.

Did they tell you when a decision would be made? Did you send them a thank you email and/or follow up email?
Just because it has been three weeks since your interview does not mean you have not moved on to the next step as the hiring manager just could be still interviewing, looking at additional candidates, out of the office, the recruiter could be just too busy to get back to you, the recruiter could have forgotten, the recruiter could be ill trained, or the recruiter could be lazy and does not really care to do the job professionally.
