If you don’t care about the potential offer going away since you seem happy in your current Role, I don’t see any harm in asking the way you noted here. I wouldn’t be surprised if they offered nothing and/or some middle ground solution at the most though, so worth thinking about what amount of leave if any would make it worth jumping ship. The other context is that this job offer isn't at my dream company (though salary increase is solid), and I'm currently employed at a company with 5 months of paid parental leave. so part of me feels that I have a strong alternative - which is just staying at my current company offering 5 months of leave, and then looking in the future. If i choose to disclose, and this future company won't give me anything in writing, I may be inclined not to take the offer.

I think your suggested wording is super smart. Given your current leave policy, you do have a real reason to stay at your current job, so it makes sense to negotiate that up front before accepting. (Currently in month 1 of my 5 month leave and I would not give it up easily. I have no idea how people make by with just a few weeks before returning to work and figuring out childcare.)
