Take your typical business casual attire and deconstruct it without looking messy or sloppy. Swap out slacks for higher-end jeans. Make sure every piece fits well and is coordinated. If you wear sneakers (creatives) make sure they're fresh and clean. Same with other street wear- street is good, but worn-out is never.
Totally depends on role and your personality. Take the same and edgy outfit and put it two different girls, one of them will look stupid in it.
Think about your personality and what you want to convey. I interviewed at Disney's and wore a really tailored outfit, all black, with these rad yellow shoes. It was conceptual! As a creative director, that's what I wanted to convey.

Additionally, and I think this is important. Ask ahead about which clients you'll be working with and adjust accordingly. If you're going to work with a cosmetics brand, demonstrate that you get cosmetics and fashion. If you're going to work with a bank, tone it down and make sure you're belted and tucked.
Ditto @CD1. I find black jeans and a (not necessarily bleak) jacket have worked out very well for me in the past. Looks nice and polished but not suit stuffy

Always wear pants. You don't wanna show up in your boxer shorts or a unitard. Unless you're really, really good.
