In your case, I would say no. Totally depends on your situation. But I’d possibly try to turn it into “medical leave” and that you decided to not return…?
I came by to say this. Don’t give notice, just go on leave and then decide not to come back.
If you get paid for FML, I'd double dip.

I would recommend reviewing your company’s policies but, generally, if you give notice in the beginning of the month your insurance ends at the end of the month.
In other words, if you give notice on November 1, you will be covered with insurance until November 30.
Great. What is your concern then? You’d have insurance coverage during your surgery, which is the same month.
Insurance end date is typically determined by your last day (termination date) not the day of notice to an employer. Meaning, if you give notice on Monday with a termination date of Nov. 8th, you’d still have insurance through November.
Is the concern that there’s a chance you’d give notice and they automatically terminate? I’m not super sure about FML, so would check that but you typically maintain benefits up to your official last day.
I’d go out on leave and just not comeback. If you tell them you’re leaving they maybe try to give you a release.
