Look at the job competencies for the level above your current role. You should be hitting at least 50% of those. In 1:1 meetings with your supervisor, ask for their support in reaching the next level. Ask for additional work. Make sure your utilization is among the highest in your office.
Keep in mind that agency fee is determined by a rate sheet or a cost multiplier. If/when you get promoted, your salary should increase (never take a title-only promotion). Are clients willing to pay more? Are they on a fixed-fee contract? What I'm suggesting is: do agency fees exist to support you in your new role? If your billings are spread across a handful of clients, it's going to be easier than if you are fully dedicated to one large client.
This is typically the reason people change agencies. It's not that you're not great. It's not that you are not worthy of a promotion. It's that the financial structure won't support it.

Can be hit or miss. I was offered money to stay as well as the promotion. More than once & at different companies. If they really value you they will find the money.

Yes, point out how you helped the department. Outside of what’s in your job description. Ex: within the first 6 months I helped make reviews more efficient. Or in Q4 during the holidays. Something that everyone is clear that you did as well as felt the impact.
Document, document, document...all your wins, no matter how big or small. Your growth, no matter how big or small. It doesn't have to be a fancy document either. Advocate for yourself because we can't rely on others to do it for us. If you hear someone (like your manager or someone on your team) say I wish we had a process document for this or this SharePoint was organized better, do it!! Take the initiative. And if the organization you are with doesn't see your value, well, now you have list of everything for your resume and interviews, if you go down that path.
