If that were me, I would refuse to fill it out. Past compensation has ZERO bearing on your interview process. And also no company provides comp info as part of employment verification. So the ONLY way they can get it is thru you. If they ask for tax docs, etc. Run
Your current pay does not play a role in your future comp. I wouldn’t lie about anything.
It’s more important to communicate your future expectations. You should do your market research and stick to what you think is your worth.
If you’re in the US, based on state rules asking for current compensation is illegal. So you should not fill any such document.

Decline politely, but be ready to answer about expectations.
They asked me to complete it too, I politely declined, they asked about expectations, I shared those and after that the interview process with their client (my current employer) progressed without mentioning past compensation again.

I guess you need to make a risk-return assessment. I assume they will not put too much effort into checking, unless your numbers become unrealistic. Disagree with the opinions here that you should claim the whole process as illegal and make a big deal out of it. I would try to play this very smooth und to your advantage.
Agreed with above. Dont disclose. Likewise talk to them about total comp. For goodness sake they are a premier HR consulting firm - they know what you're likely paid.

Sorry but fuck that. Politely decline it and just say your desired pay for the role if you know what you want and if it is within the range for the role. Your current pay shouldn’t influence your future pay but the role and your experience itself should be the main factor.

For the folks saying it's illegal...that isn't universal. It is illegal in some states, but not in others.
As of July, it was illegal in 21 states and 21 localities to ask for salary history. If you're in one of those areas, you can tell KF to pound sand and you've got the weight of the law at your back. If you're not in one of those areas, you can certainly still tell KF to pound sand, but YMMV in the results.
As a former Korn Ferry employee, we were not allowed to ask for those details. It's never s good idea to share. Show them any awards, recognition or positive feedback. I would not share any of your personal comp information.
Depending on YOUR state of residence, this could be illegal and you can avoid it. Look it up
