If you’re a “senior PM” why are you taking a job as a consultant?
Also. Where is the offer from? Is it a PE-owned physician practice? Does that align with what you want to do long term?
Let them know you would appreciate more time, and let your Deloitte recruiter know that you have an offer.

You can also probably set a start date into the future w the first offer — for like 4 weeks from now. If you get the Deloitte offer in 2 weeks then easier to back out if you haven’t actually started
You’ll probably burn a bridge, but it happens all the time tbh
If you’re unemployed then just take the offer with the offer you’ve received and continue to interview with D. If you are employed then maybe you’re willing to risk losing the other offer to continue interviewing with D.
I would accept the offer (if you’re trying to get out) and continue to interview but wouldn’t tell the recruiter about the offer I received.
It’s not going to change their process, if anything they may just decide you’re a loss and move on. Informing recruiters of pending offers is for markets where employers don’t have the pick of the litter.
I recently received 4 job offers as I continued to interview, I never once mentioned a job I recently accepted.
I would decline and wait for Deloitte. A few others are hiring in this area (check out PWC and Chartis). If you want consulting, you can get there. If Deloitte doesn’t hire you, find out where your gaps are and what expertise they are looking for.
