Everyone complains about high healthcare costs in the US. I understand that it’s a complex problem including drug prices, insurances etc. But no one talks about the high physician salaries. Why do surgeons make near a million in the US while they make $250K in Canada?

I'm US based and my salary is around $250k (NYC) -I've been asked my expectation without the startup giving me a range to work from (though I've asked). I'm not sure how to reply to them because I have a feeling that my requirement may be out of their range and while I don't want to undercut my value, I also don't want to be cut out of the process because it's an interesting co. I'm exploring roles in WELLS FARGO/JP as I am looking into profiles in investment banking. Have done masters in banking & finance, & have retail banking experience of 4.5 years. Overall 8+ YOE from scrap.
