First, good on ya for seeking feedback! That shows a genuine willingness to learn.
As someone who has been on both sides, suggest:
-start with small talk. Sounds trite but this helps ease the tension. Not an easy conversation for either person.
-thank the person sincerely for doing this, state that you were genuinely interested in the position, would like to be considered for future opportunities, and then ask what you could have done better (or need to work on).
-then shut the hell up and listen. Nod, be engaged. Take mental or real notes.
-only ask sincere questions—if something needs clarification. Do NOT get defensive.
-thank that leader at the end and follow-up with a thank-you note.
