Addendum is all you need. It can be pretty short, it just needs to mention the new end date and the new rate (if it’s different).

Just google “lease amendment” and use a free form. The terms on the existing lease should definitely be reviewed with a lawyer, the amendment usually just refers to the original agreement and says “lease end date is extended to XXX date at YYYY rate. New rate will start on ZZZZ date.”
That’s all you really need.
