I feel like I'm more or less prepared to go at any time, in that I feel pretty confident talking about my experience and how I work. I'll still do some research on any company I'm interviewing with, just so I can as more informed questions. The night before an interview I'll jot down some notes just to refresh my memory, I find that if I write things down I can remember them better. And that's about it.
Hard to say, varies from person to person and dependent on the opportunity. I can spend anywhere from a couple of hours to 2/3 days prepping for an interview. It really should be based on how long it takes for you to understand the firm, role, the interviewers, so you can make it a meaningful conversation.

It depends on the round and what are the stakes too, if recruiter screen, less prep. If a large technical or final round, then I would spend a weekend preparing
