Honestly, even if something is in writing, it’s kinda a false sense of security. Up until the day you start, there could be budget issues or other things can arise.

Are they responding to your emails/calls? If they have gone 'radio silence' then I would suspect they changed their minds. Send one last email and if they don't get back to you assume the rescinded the offer.
If they are responding, but their answers are inconclusive, contact the HM and see if they can escalate. Waiting 3-4 weeks for a written offer sounds like someone isn't doing their job.
Yes, you should never negotiate on the phone. Always wait until your first offer in writing. They can use your verbal information to find someone “cheaper”.
When a company says “expect a written offer” but never sends it, was their REALLY an offer?
I think that’s the point and spirit of what I am trying to exchange. Sometimes a hiring manager can change their mind, or find someone better through the interview process, or choose no one in the process. Until the offer is written, nothing about the interview was in stone.
HR interviewers have said to a candidates historically before, not necessarily with malice “what is your range you are willing to except?” “We really like you, and expect a written offer from us soon!” But until that offer is in writing and in the hands of the candidate it’s non-binding.

If they are pulling the offer they should send a rescind notice via email or at the very least verbally explain it. In my experience this only happens when we go on hiring freezes. Either way they should at least communicate what’s going on.
