When you say your work is constantly interrupted. Who’s doing the interruptions for the most part? Like is it your supervisor? Or is it other coworkers or patients? Because if it’s your supervisor, you can sit them down and say hey I want to be able to do this but you have to have some realistic expectations. If it’s your coworkers or patient, you could sit down with your supervisor and say hey, can you help me set up some boundaries.
Sometimes you have to block off your time and just stick to that schedule. If that means closing a door or going somewhere that people can't interrupt you, that is what you have to do.

It’s so hard and why work from home is so helpful. Try putting on very visible headphones and maybe a sign on your computer saying “focus time.”
Have you tried discussing this issue with your boss? I feel like that’s the first step to getting to the core of the problem.
Who’s interrupting? I have a sign I put on my door and close the door if I have stuff I absolutely need to get done. It has an end time to when I will be available and says to knock only in emergencies.
