That sounds like an awful environment. Brainstorm about what you really want to do, and start looking for a better job. There has to be something out there for you, just get serious about looking.
You're an Adult. You have options. Quit and ROAST her in the exit interview. Or you can stay and be miserable.

It definitely sounds like a toxic work environment you're currently in. You definitely deserve better than that. You should definitely look for a new job ASAP
I am sorry you are having these issues. I think they may be exaggerated. Some of your complaints, like the difference between vacation and PTO hours, is basically semantics. I doubt that inferior employees are kept and rewarded while good employees are not. Micromanaging, whether it be from afar or standing over your shoulder, is irritating and demeaning. Unfortunately, many managers think that's how they are supposed to do their job. If they had a micromanaging supervisor when they were in your position, they think that's the way to do the job. That doesn't help you, and it certainly does nothing to "break the cycle of abuse," as the social workers might say. I would think the decision on whether to pay for training or allow online training courses to be taken on company time is probably handed down to her and is not her idea. If that is a major issue for you, submit a request to HR or the hospital CEO or Board of Directors to consider adding that benefit for employees. It would help them and it would benefit the company. You have two ways to go at this crossroads: stay and try to make it better for others or leave and do what's best for you.
I am really sorry to hear that OP. I think if I were you I would start looking for something else. Especially if you are being told that you don't get any vacation. There are plenty of places that are better to work and will fully train you, time to start appying.
