So, how do you tactfully tell a patient they're overweight and need to change their lifestyle? The internet seems to have convinced young adults that being overweight isn't actually a health issue and that all of us doctors are just... fatphobic.

At everyone's physical I ask how often they eat veggies, fruit, legumes, and whole grains. I ask how often they eat fast foods and drink pop/juice eat sweets. I ask how often they exercise. All adults are supposed to get 150 minutes of moderate intensity exercise weekly and 2 days per week of strength training included in that. If they aren't meeting the requirements I give them the recommendations and try to tailor the recs to their lifestyle. I document this as a part of my social history.

You also have to assess readiness to change and sometimes you have to build rapport with a patient before they are ready to take advice.

It’s all about readiness to change. People that are overweight know they’re overweight. If they want to change they’re open about that discussion if done tactfully. Breaking out the BMI chart and ideal weight chart do more harm than good. The 300lb man knows he’s overweight, trust me. Saying “based on xyz you should be 180” is not going to help him. He’s going to say no shit Sherlock and walk out of your office annoyed.

Focus on common ailments that might be exacerbated by weight- poor sleep quality, heavy breathing, getting winded quickly, not being able to keep up with family or at work, etc.

Ask them where they see areas for improvement, what’s their biggest concern, what risk factors they have and their perceived risk, etc. Obesity can impact just about every health issues and area of life. Let it come up naturally, help them set realistic goals. And for goodness sake- have a registered dietitian that accepts their insurance in your referral pool!!!

When we focus on weight, we are only acknowledging that we don’t have enough time to talk about all the other things. Dietitians recommendations are spot on, and the only real change happens the incrementally and is driven by feeling better in their body. We have to stop talking about weight and start talking about Exercise tolerance, well-being, energy level strength, sleep apnea chronic pain. Helping patients make tiny incremental changes is the only way we’re going to help them achieve sustainable success.

Yes. What the dietitians said. It’s a reflection of having no time to spend with patients that we feel the compulsion to focus exclusively on weight. Focus on literally anything except weight. Patients know they’re fat. Rubbing noses in it just drives folks more towards their familiar coping skill…which is likely food.

Starts with (communication style) being loving and welcoming to the patient. Introduce the topic by saying so we looked at your blood work and all would you please tell me about what you usually do on a daily and what you are drawn to eating everyday? If you notice their unhealthy diet start inviting healthier options and informing them to be aware of getting good social support around them as eating is also emotional, so to stop the emotional eating you need to heal the underlying issues for them to feel comfort and progress in their diet and lifestyle changes… Hope this helps!

I know that some other associates at my level are approximately 30% behind other associates in hours on a month to month basis.
