I always ask about turnover rates including tenure of the team I would join. I also ask if the position is new or a replacement. Sometimes I even ask “why do people typically leave ?”I believe that’s telling.
Oh that's a really good question! I will be keeping it in mind for future interviews. I would be curious to know how some past companies I worked at would answer that question.

From personal experience, look for the words fast paced. I’ve said this in other posts and I say it again, that’s a big red flag that work will be short handed with lots of overtime and you need to dedicate your life to them.
If they say “we work hard and play hard,” run. If no one can answer why they personally like working there, run faster. Interviews are often rehearsed—learn to read what’s not being said.
"We're a family here" is always a major red flag. I like to ask about their system for reviews and raises. I find if they are uncomfortable about being asked then chances are they will always pay you the lowest possible amount they can get away with. A good company recognizes that you work for money, not for fun, and will be comfortable answering the question.
I totally agree with your approach! I also look for how much they emphasize collaboration versus competition, if they focus too much on individual performance, it can be a red flag. Green flags for me are when they ask about your long-term goals or how they support career growth.
I would add to the first point that you need to watch out if they talk about the person who had the job before you as if they were the cat's meow. This sets you up for failure if you cannot perform as they did. They will always be comparing you.
