I am prepping for my interview specifically GCA and open ended questions. I struggle to tell if the outcome should be talking through your process/steps (maybe for a question like - how would you go about determining program complexity) vs a specific solution such as create x new product, for x market by x date based on my assumptions. Is the desired outcome something I can clarify with the interviewer?
Hello.. i have 6 years of experience in frontend development. I have offer of 27.5 from Nagarro (client to be decided yet) and 30 from HCL Technologies (apple client and work location also apple) and 26 from Netcracker Technology Corp. maybe with some joining bonus. Which do you think I should join.

Typically yes, it's an instant reject unless you absolutely crush your other three interviews, as mentioned above. What's "crush" mean relative to "passing"? It means you'd have to have the other three interviewers pounding the table for you and the fourth saying you weren't great but they could see passing you. So again, it's unlikely but not impossible.
And A1 above may think s/he bombed, but we're all terrible judges of our own performance in high stress interviews. They clearly did well enough to get an offer.

No. I bombed one McKinsey final round and got the offer. It’s better to crush two and bomb one than do ok in all three. They’re looking for your ceiling
Update - this sorry for lack of context here - this was a friend, not me - she did not get the offer
I’m worried as I cased with her and we both were at the same level (have mine coming up tomorrow for a phone interview - hoping for the best)

no. don't think so.
It depends if it’s something coachable. There is a minimum expectation on calculations expected but if the thinking is right behind it you’ll be fine. Will depend on how well the first few performances are.
Good luck and know that you can always seek out feedback from each interview phase
I’ve passed people, even if the calculation was off as long as they had:
1. Clearly structured thinking and good insights on the math- a clear “feel for the numbers” and did a mental check about whether the numbers made sense
2. did very well ( not just bar clearing) on the rest of the interview with me
3. Did very well ( not just bar clearing) on the rest of the interviews with others
