Q48+ is usually considered safe no matter your background.
It doesn’t work like that. It depends on the rest of your profile. For example, M&A or data analytics consultant with 4.0 in math and a CFA, probably fine. Human capital consultant with a low gpa in a social science major with bad grades in quant courses, 49 might be iffy in terms of the whole picture of your quantitative ability. Furthermore, if the rest of your profile is great, they may still admit you even if they doubt your quant ability or vice versa.
Mentor
There is no hard cut-off, and as D1 explains, much depends on the rest of your profile. Can tell you from personal experience that M7s are attainable with a Q42 for an ex-consultant. Past performance is no guarantee of future results, of course.

I got Q46/V50 and got in to a few M7s for what it’s worth - not sure how much they actually care about Quant given they don’t really report it
Got in with a 47Q. I think the total number matters more, but more importantly it’s about your application as a whole, not just one number on it. Tell good stories in your essays, choose good recs, have ec’s that are meaningful.
Q49 and got into HBS
