Cleared Accenture Skill interview by answering almost all the questions and received congratulatory mail for HR round and submitting all documents. Then after a week received a call from HR saying that due to some technical issue, the skills round will happen again and an online assessment was set up. In the assessment, questions which were not directly related to the profile were asked and then a rejection mail was received. Does this happen a lot?

It varies from firm to firm...something like bdo, rsm, gt and all the smaller non bug 4 firms are still massive, You have to go real small but not too small because you want decent benefits and you want there to be enough staff that you are not doing EVERYTHING... I worked for a firm that had about 100 employees, I was on the audit side with a bit of tax prep.. it was good, busy season we were out of there by 7 on most days. The rest of the year out at 5:15 every day like clockwork and also got a 1hr lunch break which I actually took.
You will never improve your career by going to a less prestigious firm, you will actually hurt your career going to firm nobody knows...you will send resumes to companies and they will immediately pick the one that has a big name firm or company go interview just because they recognize it. Nobody wants to interview someone from random, random & associates, llp.
^Depends on what you define as improve. Plus who says you have to take B4 off your resume even after 3rd or 4th job? I would advise mid-size if you're not sure where you want your career to go. But, if you find the right small firm, the personalities click and you're happy, that's a lotto win in my book.
Trying to make the decision between midsize and smaller. Is a much smaller firm a bad career move or does it not make a big difference since neither is Big 4?
