Help - how do I fight claims by scammer healthcare providers?

I recently went to 2 different providers.

Dentist, made appt via Zocdoc to see a dentist that was in network. Another dentist at the location saw me instead who was out of network. I have text evidence they said it would be covered before visit. Claim was denied - asking for ~500 dollars.

Saw a specialist who’s office staff verified my insurance would fully cover the claim. Get the claim, says they are out of network. Owe 600.

Told a friend of mine in tech I was considering moving to BCG. He asked me if I would have to move to Boston.

I followed up by asking what he thought the top consulting firms were. "I'd guess PwC and then McKinsey are the best. I've also heard of KPMG, Deloitte, and Bain, but I don't know much about them."

I don't follow pop. culture anymore, no longer actively follow team sports (but sporty myself) as I consider it bread and circus for the masses and don't rly enjoy work drinks... Am I doomed? Alright, fishies, let’s try this again. Name drop boutique firms that have offices in San Francisco Bay Area. Big 4 is not my future (as many many MANY of you have repeatedly said I’m not good enough for). So, I’m taking the hint and crossing MBB and Big 4 off my list but still hoping a smaller firm will consider me.

Have junior/mid-level (Manager and below) consultants at your firm escalated long working hours as a major issue in the last ~6months? Does your firm believe it is a legitimate issue? How is your firm winning and delivering solid client work while dealing with this issue?
