Brand consultancies have very rigid processes and frameworks. It’s somewhat similar to how management consultancies do things, but yes more tailored to comms.
Most ad agency planning teams draw the three circles, have a brief template and call that their process... what they don’t get is that clients prefer to buy the very important strategic thinking from an agency that can talk about process too (whether or not they follow it in the actual work).

Depends on client and the challenge. Most agencies have a few tools they always roll out but it’s far less uniform/repetitive in my experience.

Aside from firms at the caliber of a Prophet or Lippincott, most other brand strategy firms aren't using such robust frameworks and methodologies that are similar to those in the management consulting world.
You know, the prophet work I’ve seen was extremely mediocre. I’m sure it wasn’t entirely them (client needs to shoulder some of it) but it was all just highly disappointing.
Aside from t mobile (the value creation of which is debatable) what else have they done?
