I’ve never been given a chance to negotiate my salary at the time of promotion - was just given a paper signed by HR. How did you get the opportunity to negotiate?

If they were planning to give you a raise, they wouldnt have left that part out. Youre welcome to give it a shot, but its a pretty standard move to give someone a title bump to give them the illusion of progress, without a change in money or daily responsibilities.
To be cagey about it, start by asking innocently how your duties will change. Will you be managing people? Left alone on bigger projects? Given higher budgets to oversee? Yes? “Sounds like it would be fair to talk about a higher salary then?” No? “It doesnt sound like anything is changing from my current title then, Im curious why promote me now?”

Yes, always. Being offered a promotion and a raise is a clear signal they want you to stick around, which is one of your few leverage points as an employee. Frame it like "I'm grateful for the opportunity and responsibility, and I love working here and would love to stay here long term, and this is the amount I think is fair given my contributions, amd that would make me happy to stay committed to the team." They might say no, but they'd be doing so knowing you might get scooped up by someone else. They wouldn't promote you if they weren't at least kinda worried you'd be scooped up.
What’s the standard amount of raise if you are going a title up?
Yes always. Be direct and talk about what additional responsibilities your taking on, and why you're asking for X amount. Do your research and share tangible results/proof why you're deserving of it.

You do it when you ask for a promotion. I’d like to be x title and compensated y. If you’re being given surprise promotions, well, good for you.

Was recently given a promotion from senior to ACD but with an internal transfer to another company. No salary was discussed before the offer letter. I received the offer letter and made a meeting with HR to discuss it. You should always negotiate. Know the market, know your worth. Add that within your new role this is how you will bring more value to the company and an amount XX is fair and comparable given the current market. Keep in mind,
internal promotional bumps won’t be the same as if you go to another company, however it is possible to negotiate more depending on the company. It doesn’t hurt to try!
