No this has never happened to me. Although if I had a vacation scheduled I always told them during the interview, not after an offer had been made. But generally places are pretty understanding so I am sorry that happened to you.
When would you recommend during the interview process to mention it and should
It be said in email?
Personally, I haven't experienced this but it can be a bad look for the new job if you inform them much later after starting the job. I once disclosed in an email with my manager at the time, while we were firming up my start date and it was not an issue. I got the time off but they stressed it would be unpaid because no PTO earned. I was fully prepared for that. Being upfront about it as early as possible after you have an offer in writing would be the best course of action. But make it sound like you’re asking for flexibility rather than demanding.
In my instance, I let them know right after receiving and signing the offer and they still rescinded.

Really sorry that happened to you.
It sounds like they made a cold business decision.
Like others said, perhaps informing them immediately after they extend an offer would be better timing
This just happened to me. And mine was only for 6 days. I even said it was tentative so I was willing to work with them. Rescinded after 24 hours after they went on about how perfect I was for the role. Makes me think there was another reason behind it and they used the vacation as an easy out.
