I did it, but not soon enough. I suffered burnout so extreme that I lost myself and ended up destroying my career. If you need the time take it. Go see your Dr. Tell them you are experiencing burnout, if they okay the leave then just ask HR for the short term leave of absence paperwork.
I did two years ago. I was grinding to a halt and just couldn’t go on. I took about six weeks off for medical leave under FMLA. The HR people made the process more confusing than it needed to be but it all worked out and management were much cooler than I thought they’d be (though I never stated the condition I was experiencing).
I did fear how it would affect my reputation—I didn’t want to be known as “the sick guy.” But there was no reason to worry. It was the right thing to do to get the care I needed and I’ve been back at work for two years now without issue. It will feel a bit awkward, but please do what you need to do.

I am currently in FMLA leave. Had to undergo 2 surgeries but my workplace is also super toxic. I return to work next month as my 12 week FMLA leave gets over. I am scared and worried that they might fire me because my manager was giving me the silent treatment before I went on FMLA. I really needed the FMLA leave and I am glad that I took it. Also interviewing at other places in case my employer decides to let me go.
I did in a previous role! It was so incredibly worth it (I was super depressed and not functional) I was one foot out the door anyways so I didn’t give much of af what my supervisors thought anyways, but I didn’t have any issues returning or leaving even though I gave everyone ~1 day of warning. One pro is I think me taking leave forced leadership to back off my team a bit :) feel free to pm me if you want to chat process specifics
