I have a Sr. Director title but it's for a small company. In the future if I decide to move on should I accept a manager or Sr manager title a large enterprise company or does the skill set translate well?

I've become really jaded about consulting. I just left my pre-seed startup that was running out of cash (we made $100,000 in revenue this year, and sunk $1m in costs). I was making $40,000 as a developer (plus 20k stock options at $1/share per year) and they refused to pay me any more money. I have 17,000 vested stock options, total number of shares at the time of my contract was 4million. Thinking of telling my manager I need to do part time to take care of parent recently diagnosed with chronic condition. This isn’t true, but thinking of using this as an excuse. I thought HIPAA would allow me to be vague like this, and it’s jerkish of them to say no.
