I have applied for 3 wellness leaves next month together. Will I need to submit any medical certificate?

So, recently got put on a PIP after six months at my new job., two days after I told my boss and HR I recently learned I have a cognitive disability caused by an illness. The PIP was mostly because I forgot some deadlines, but also they said I need to be more professional in emails (even though others use zero punctuation and a lot of emojis. To clients.) and simultaneously ask more questions and ask fewer questions. Should I just assume this is unattainable and look for a new job again?

So, my MS got caught as a subcontractor for one of my vendors and he was doing their work instead of the property's while clocked in I brought it to HR. we did a cease and desist despite my objections...MS ended up putting his two week notice during which he didnt work and spoke ill of me and my remaining staff to residents...today was his last day and with my regionals permission i told him that our vendor was not allowed to use him on our property due to the conflict. l interviewed for Deloitte (India not USI) in January, and accepted an offer letter with DOJ of 25th April, later HR asked me to join on 11th April for which I agreed. Now I haven't received the revised offer letter or any links other than BGV and Pre-Hire form from Deloitte and now the HR is not even picking my calls or responding to emails.
