I get that it's uncomfortable. But this is one of many uncomfortable situations you'll have to deal with (like if a client doesn't pay for example). If it was promised via email, that's easy. Reply to that email asking if they need anything from you to get it in place. If verbal, then you have to just have the tough convo with your manager - you were promised a raise. It is very reasonable to ask when it will take effect so you can plan financially.

I would keep it simple: "Manager, I wanted to follow up on our discussion in [month] about my promotion to [title]. I'm really excited about it for the reasons we discussed and ready to hit the ground running in my new role. When will that go into effect?"
Tweak as makes sense for your specific circumstances. Eg, if they promised it to you a specific month and that month has passed, I would remind them.
