Why would you need to disclose this at the application level? I don’t believe you need to and the question would be illegal to ask in an interview.

If you need accommodations, you should definitely disclose it. And yes, it’s for HR only. I’ve never gotten those disclosures in all the years I’ve been hiring.
A request for accommodation is separate from EEO questions. Typically there is a section at the end of applications to voluntarily self ID with gender ethnicity veteran and disability status. Those responses are anonymized and for government reporting. A disclaimer will ask if you have an accommodation request, that will go to HR/TA to execute on the accommodation needed. You do not have to disclose any disability, just what you need to support you during the interviews. ie: you have a learning disability and need extra time during a technical interview. No need to disclose learning disability, just that you will need x amount more time accommodation for that portion of the interview.

No, why do you feel the need to disclose your private affairs? No not for HRs eyes only. HR job is to avoid and eliminate business disruption. You sharing something personal as such at job interview could sen as that. Not everyone is open minded to the idea.
