Oh, I hear you. I am so guilty of this myself. Whenever I finally do take a vacation or give myself a break work is always on the back of my mind. And then I find myself checking emails or Slack. I don’t know how to claim my own time. Seems like work stress is just always with me

I wish I knew. I respect the time off my employees and people I work with have, but I feel that I don't get the same courtesy. The worst is when I get a message saying, "I know your are on PTO, but..."
I have more hours than I'm allotted to carry over, but I'm hoping they pay it out because they knew how busy I was this year.

Agree with a golf trip or something like that. Could even be just focusing on hobbies you have, and if you have none, the perfect time to start one! For me to stop thinking about work it really is a matter of making myself focus on something else and giving me that break from it.

Ask yourself why you feel so bad then take a trip. Nowhere in particular, just fill the gas tank and head ( west or wherever ) just get away. Break the journey when you hit a pleasing vista, river or town.
